The above patient had a long standing history of ptosis, noted by the eyelid drooping and covering the pupils of the eye.  Also, on the left hand photo the distance from the eyelashes to the eyelid fold is long and the eyelid fold looks very high. This indicates the muscle attachment to the eyelid tissue is improperly high and needs to be reattached lower, closer to the eyelash area.

The photo on the right shows the effects after shortening and advancing the Levator Palpebrae Superioris towards the eyelashes. The eyelid level has been raised, allowing visualization of the pupils and the eyelid fold is now closer to the eyelash line indicating a proper attachment of the muscle to eyelid tissue.



Blepharoptosis is a dysfunction of the elevator muscles of the eyelid.

There are two muscles that elevate the eyelid

Levator Palpebrae Superioris

Mueller's muscle

Causes of ptosis (drooping eyelid level) - are generally due to factors that effect the nervous system or the muscles themselves.

Tumors

Trauma

Neuromuscular diseases (e.g. Myasthenia Gravis)

Laxity of the muscle elevators from stretching or aging

Hollowness in the outer lower eyelid area sometimes requires lifting of the cheek pad in addition to the lower eyelid Blepharoplasty.

Lower eyelid incision - this can be used to lift the cheek or mid-face by anchoring to the bone around the eye (orbital bone) an Endotine Bioabsorbable Anchor.
